To receive a win/loss statement from the Eldorado Resort Casino in Shreveport:

• Fill out the Online Win/Loss Form and we will mail you the statement.
• Call 1-318-220-5259 and leave a voice mail with your name, Player Card account number and address, and we will mail you the the Win/Loss Statement Request form.
• Download and print out our Win/Loss Statement Request – you will need Adobe Acrobat Reader to print the form.
